Patient Info Form
Before your visit, you’ll complete a new patient form so we have a complete record of your medical history.
For your convenience, you can complete and submit the form in advance.
Consult
Dr. Matt will conduct a thorough consultation with you to learn about your health history, current symptoms, and most importantly, your personal health goals.
This helps us understand not just where you hurt, but what you want to get back to doing.
Exam
Dr. Matt will examine your spine and test the range of motion in your joints.
This step helps us evaluate movement, function, and areas that may be contributing to your pain or discomfort.
Digital X-Rays
Dr. Matt may take digital x-rays to immediately assess your spine and further analyze your underlying condition.
Not every patient needs x-rays, but when they are helpful, they give us a clearer picture of what may be causing your symptoms.
Recommendations
You’ll see your x-rays and receive a thorough analysis and report of findings on your next visit.
Dr. Matt will also outline your treatment options and make customized recommendations for achieving your goals.

We do not want finances to keep you from getting the care you need.
Coastal Chiropractic accepts nearly all PPO insurance plans. We also offer very affordable multi-visit cash payment plans for patients who prefer or need a self-pay option.
